Participants can register using the Online Registration Form or by printing out the Registration Form for use by fax or mail.

If you are not pre-registered and you wish to register on-site you are cordially invited to do so during the entire Dystonia Europe Congress
from October 17- 19, 2008. You may pay the conference fee as listed below either by cash or via your credit card (VISA, EURO, MASTERCARD)
directly at our Conference Office and during the official Office opening hours:

Congress Material
You may collect your complete Congress material, including the Conference programme, name badges and Gala Dinner ticket (if purchased) at the Conference Office which is situated in the Foyer of Hall G (2 nd Floor) of the CCH – Congress Center Hamburg during the official opening hours (see above)
Payment
Bank Transfer
Our General Terms and Conditions apply, which you accept with the booking order. The payment must be made together with the registration, at the latest within 14 days after sending the registration form. Please remit the total amount in Euro for the registration fee, workshops, and include the cost of the gala dinner and/or hotel deposit if appropriate, to the INTERPLAN account, as detailed on the registration form.
All bank charges must be covered by the participants directly.
Credit card
Please indicate on the registration form your credit card organisation, credit card number, expiry date, CVC-number (last 3 digits on the reverse of the card) and name of card holder.The total amount of the conference fee/ workshops/gala dinner will be charged to your credit card at the next possible date. Your signature constitutes a declaration of consent for the conference office to pass on your credit card data to the hotel, as a guarantee of the hotel reservation. This means it is not necessary to make an advance payment to the hotel.

Hotel Wagner - 3 Star
The Hotel Wagner is situated in the centre of Hamburg and within easy walking distance of Dammtor Train Station and the adjacent Congress Center. The rooms are equipped in a modern style and decorated with photo impressions of Hamburg. Free of charge public parking is possible right in front of the hotel. 24-hour check-in service is available as well as baggage storage.
Hotel Amsterdam - 3 Star
This hotel is also situated in the direct surroundings of the Congress Center (CCH), 10 minutes walking distance to the city and to the Alster Lake. It is a modern Bed & Breakfast Hotel with an individual style in a beautiful, old building classified as a historical monument.
Hotel Alster-Hof - 3 Star
The Hotel Alster-Hof lies directly in the city and only a very short distance from the Congress Center and Dammtor Train Station. The rooms are traditionally furnished and some rooms have a connection for internet access.The hotel offers a new fitness suite, with a sauna and solarium. There are several restaurants close by.
Hotel Baseler Hof - 4 Star
The Baseler Hof is next door to the Alster-Hof and only a short walk from the Congress Center and Dammtor Train Station. The rooms are furnished in a modern style and offer all the convenience of a 4-Star-Hotel. The fitness area includes sauna, steam bath and fitness room. The hotel has a restaurant and a wine bar.
Hotel Mercure an der Messe - 3 Star Superior
The Mercure is within walking distance to the Congress Center. The rooms are airconditioned and triple glazed and have WiFi wireless Internet access and a high speed transmission line.The Olive Tree Restaurant offers Mediterranean cooking and the Lemon Bar offers snacks and cocktails. The hotel has private indoor parking.
Hotel Grand Elysée - 5 Star
The Grand Elysée is in walking distance to the Congress Center and Dammtor Train Station. All rooms are furnished to the highest level, have triple glazed windows and offer all the convenience of a 5-Star-Hotel. It has a luxury health and spa area, and several restaurants offering excellent cuisine. The hotel has its own garage.
Radisson SAS Hotel - 4 Star
The Radisson is the tallest hotel in the city and provides direct access to the Congress Center.The rooms are tastefully decorated in a timeless and functional style and are equipped with the amenities expected of a first-class hotel.
